PROGRAM OBJECTIVES

The Indigenous Mental Health and Outreach Program is established to:

  • Do outreach in Scarborough and engage Indigenous communities in assessing what is needed to provide support
  • Provide one-on-one support, referral and case management
  • Develop partnership with other indigenous service providers and community groups to implement culturally appropriate programs and services for community members
  • Liaise with hospitals and facilitate transition from hospital to home and community

Build capacity of organization to better serve the Indigenous communities through training and awareness.
